Home Care Packages

Program helps older community members to stay in their homes and live an independent life for as long as possible. Services are tailored to individual clients’ needs. Program run by the Polish Community Council of Victoria.

Program provides a range of entry-level aged care services for people aged over 65 who need assistance to keep living independently at home and in their community.

Polish Community Council of Victoria Inc.  are currently participating in ELDAC Linkages, a sub-program of End of Life Directions for Aged Care (ELDAC, www.eldac.com.au), a national project funded by the Australian Government Department of Health to develop and support palliative care and advance care planning in aged care.
